This document will walk you through how to attach a document (like a doctor’s note) to a time off request. This is for after you have created a request and it has been approved by your supervisor.

  1. From the main screen in Employee Access after you have logged in, click on the “Time Off” tab and then on the “My Requests” link.
  2. Highlight the day you would like to attach the document to by clicking on it.  Then click the “Attach” button.

     
  3. On the next screen, click on the “Add File” button.

     
  4. On the final window, click on the “Type” drop down.  You will only have one option.  Click on that then click on the “Choose File” button.  This will open a window to browse to and upload the file of your choice.  After it is select, click on the “Save” button to finish.  You can add some further description (if needed) in the description box before clicking save if need be.
